The polyphase proc is gained at the start of combat.
Reducing rage and control
The amount that an opponent's rage and control are reduced by depends in part on the polyphase proc's level:

As of 9 July 2013, no current epic boss can gain the polyphase proc in battle.
The polyphase proc can be gained by any coven boss during any battle.
